First, complete the form below with information on all members of your household. Then, email the following required documents to [email protected]:
Current Government Issued Photo ID for all adult members of the household.
Birth Certificates for all household members under the age of 18. Passports are not accepted as verification of birth date.
If you are a Hallandale Beach resident please provide two proofs of residency no more than 90 days old (current utility bill, etc).
Please note that Florida has a broad public records law. All correspondence via email may be subject to disclosure. If you prefer, you may bring your documents to our administrative office at 410 SE 3rd St, Hallandale Beach, FL 33009. We are open Monday - Thursday 8:00am - 5:30pm.
